Position Overview
We are seeking a skilled and dedicated Physical Therapist (PT) to join our dynamic clinical team. In this role, you will provide essential rehabilitative services to identify, prevent, and treat acute or prolonged physical dysfunction and pain.
Serving a diverse patient population spanning from pediatrics through geriatrics, you will utilize comprehensive evaluations, therapeutic exercises, and advanced physical agents to maximize functional independence, accelerate healing, and improve overall quality of life.
Key Responsibilities

Patient Evaluation: Conduct thorough initial assessments and evaluations to diagnose physical dysfunctions and establish personalized treatment goals.

Therapeutic Intervention: Design and implement evidence-based rehabilitation plans utilizing therapeutic exercise, manual therapy, and physical/chemical agents.

Lifespan Care: Provide age-appropriate, high-quality rehabilitative care to patients ranging from pediatric to geriatric populations.

Progress Tracking & Documentation: Monitor patient progression, adjust treatment plans as clinically indicated, and maintain accurate, timely electronic medical documentation.

Collaborative Care: Partner with multidisciplinary healthcare teams, patients, and families to ensure safe, continuous, and effective care transitions.
Qualifications & Requirements
Education & Experience:

Required: Graduate of a physical therapy program from an accredited school with a degree in Physical Therapy (DPT, MPT, or MSPT).

Experience: Previous clinical experience is preferred, but passionate New Graduates are highly encouraged to apply!
Licensure & Certifications:

Required: Must possess and maintain a current, unrestricted Physical Therapist license in the State of Illinois.

Required: Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross.
