Are you the One? We are looking for an
Environmental Modification Coordinatorwho will provide coverage and support to environmental modification providers and waiver participants across the state of South Carolina. The position collaborates with Community Long Term Care (CLTC) case managers and Medically Complex Children (MCC) care coordinators to evaluate environmental modifications, enabling participants to function with greater independence and remain in the home. This position involves traveling throughout the state to include overnight travel to various regions within the state. This position reports to the OIDD and Youth Waiver manager in the Bureau of Policy. Collaborates with OIDD environmental modifications staff as needed.
- Assists with providing oversight to enrolled environmental modification providers and reviews provider bids. Ensures modifications are necessary and related to the assessed Activity of Daily Living (ADL) or health-related needs of the individual and are tied to a goal in the individual’s written Plan of Care (POC). Conducts on-site inspections of environmental modifications. Ensures work meets all applicable state and local building code requirements. Recommends a corrective plan of action for unsatisfactory work.
- Notifies CLTC Central Office, MCC waiver staff, case managers/care coordinators (as appropriate), and Bureau leadership of reported deficiencies of enrolled environmental modification providers. Participates in developing solutions to correct identified deficiencies. Works effectively with clients, families, care providers, contractors, and internal teams. Facilitates and manages issues related to the provision of authorized services with the support of supervisor if necessary.
- Provides specialized knowledge and expertise related to environmental modifications necessary to ensure the health, welfare, and safety of the waiver participant. Conducts assessments and makes recommendations for modifications that enhance independence and safety. Ensures planned modifications comply with applicable regulations and guidelines. Educates waiver participants about the home modification process. Works closely with case managers and care coordinators on recommended modifications.
- Maintains knowledge of the Environmental Modification Standards found in the Home and Community Based Services (HCBS) Manual and the environmental modifications allowed for each of the South Carolina waiver programs (Community Choices, Community Supports, Head and Spinal Cord Injury, HIV/AIDs, Home Again, Intellectual Disability and Related Disabilities, Mechanical Ventilator Dependent, and Medically Complex Children). Participates in administrative fair hearings, as necessary.

Minimum and Additional Requirements
Bachelor’s degree and relevant professional experience in social service programs, business administration, general administrative management or relevant program experience.
Additional Requirements:
- Occasional overnight travel.
- Requires holder to drive routinely.
- A valid driver's license is required.
Preferred Qualifications
- Licensed residential contractor with knowledge of state and local building codes, contractor and procurement rules, and the Americans with Disabilities Act (ADA) Accessibility Standards.
- Understanding of South Carolina waiver programs and environmental modification regulations.
- Understanding of South Carolina waiver programs, eligibility criteria, and environmental modification regulations.
- An average of two days a week of overnight travel is necessary
